At Inova, caring for our patients extends far beyond just addressing their medical needs. As a leading healthcare system, we must also acknowledge and address the impact that societal factors have on patient health by screening for social drivers of health. Social Drivers of Health are the conditions in which people are born, grow, work, live and age that can shape their health outcomes. To help address these health inequities, Inova has a tool available in Epic to document information on Social Drivers of Health (SDOH). This video explains what these drivers are and why it is important to fill out this information when visiting Inova.
