It was an accident waiting to happen.<br/> <br />Five children were killed and more than two dozen injured when a train and school bus collided at an unguarded railway crossed in northern India.<br/> <br />Residents of the town in Uttar Pradesh say they have been demanding that authorities install a gate at the crossing, but that never happened.<br/> <br />Injured children were rushed to a local hospital where parents and medical workers tended to them in a chaotic scene.<br/> <br />Road and rail accidents are common in India which has a notoriously rundown infrastructure.<br/> <br />Prime Minister Narendra Modi has vowed to fix it, but that promise comes too late for the victims of Thursday's accident.
