The National Transportation Safety Board concluded that two fatal school bus crashes resulted from poor oversight of drivers.<br />According to NTSB chairman Robert Sumwalt, the drivers are not solely at fault for a pair of crashes in 2016 that led to the deaths of 12 people, including 6 children.<br />Sumwalt added that in both instances, poor driver oversight by the school districts and motor carriers were partially responsible. <br />
