APGAR newborn assessment score scale (made easy) is a quick test used to assess the status of a newborn after birth. APGAR is used to assess a newborn at 1 minute and 5 minutes after birth. If the score is 6 or less the newborn will be assessed again at 10 minutes after birth. The word APGAR is a mnemonic for the categories assessed during the APGAR score assessment. The letters of APGAR stand for the following: Appearance (skin color), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), Respiratory (effort). Each category is scored 0-2 and the higher the score the better the baby is doing. A score of ten is the highest score a newborn can score. For the NCLEX exam and lecture exams, the nursing student should be familiar with the nursing interventions for a newborn based on an APGAR score. APGAR scores 7-10 require normal post-delivery care, scores 4-6 require some resuscitation measures such as oxygen/suction and stimulating the newborn, and scores 0-3 require full resuscitation measures.
