<br /> The safety driver of a self-driving Uber car in Arizona was streaming a TV show at the time of her fatal crash.<br />Reuters reports that a police report deemed the March 18 incident “entirely avoidable.”<br />The Tempe Police Department said the driver, Rafaela Vasquez, hit Elaine Herzberg, 49, at night.<br />Vasquez could face charges of vehicular manslaughter.<br />The Uber car was in autonomous mode at the time of the crash. <br />
