Minnesota Strips 800 Providers From Medicaid in Sweeping Fraud Investigation

Minnesota has begun a large-scale cleanup of its Medicaid provider rolls, removing roughly 800 inactive providers by October 2025 in the first phase of a broader program-integrity effort.
