A UN report into the assassination of Benazir Bhutto, Pakistan's former prime minister, has blamed Pakistani authorities for her death. <br /><br />The head of the UN inquiry said Pakistani police deliberately failed to thoroughly investigate Bhutto's death, adding that security measures taken to protect Bhutto had been "fatally insufficient". <br /><br />The report also said Bhutto's death could have been avoided and it called on Islamabad to bring her assassins to justice.<br /><br />Al Jazeera's Kristen Saloomey reports from UN headquarters in New York.
